Requirements:

1) Submit documentation showing a minimum of 50 clock hours of community service performed during the 2016 calendar year. This documentation must include the contact information of the agency for verification. In addition, documentation must be signed by the person in charge of the agency or event confirming that you have completed the stated amount of community service hours.

2) Submit an official copy of your high school transcript which must include your ACT scores. You must have at least a 3.00 cumulative GPA on a 4.00 scale to apply; and

3) Complete and submit the attached application. Each of the aforementioned items must be emailed to Alphasigmazetascholar@gmail.com or mailed to the attention of the Scholarship Committee c/o Doris Hardy at the address on the application and must be dated and time stamped or postmarked by Tuesday, January 31, 2017, to be considered. Incomplete and/or late applications will not be considered.
